Background: Decisions of patients, families, and health care providers about medical care at the end of life depend on many factors, including the societal culture. A pan-European study was conducted to determine the frequency and types of end of life practices in European intensive care units , including those in Israel. Several results of the Israeli subsample were different to those of the overall sample.Objective: The objective of this article was to explore these differences and provide a possible explanation based (...) on the impact of culture on end of life decision making.Method: All adult patients admitted consecutively to three Israeli ICUs who died or underwent any limitation of life saving interventions between 1 January 1999 and 30 June 2000 were studied prospectively . These patients were compared with a similar sample taken from the larger study carried out in 37 European ICUs. Patients were followed until discharge, death, or 2 months from the decision to limit therapy. End of life decisions were prospectively organised into one of five mutually exclusive categories: cardiopulmonary resuscitation , brain death, withholding treatment, withdrawing treatment, and active shortening of the dying process . The data also included patient characteristics , specific therapies limited, and the method of SDP.Results: The majority of patients had treatment withheld, none underwent SDP, 62 received CPR , 31 had brain death , and 18 underwent withdrawal of treatment . The primary reason given for limiting treatment was that the patient was unresponsive to therapy . End of life discussions were held with 132 families , the vast majority of which revolved around withholding treatment and the remainder concerned withdrawing treatment .There was a statistically significant association between the type of end of life decision and region—that is, the northern region of Europe, the central region, the southern region, and Israel.Conclusions: Regional culture plays an important part in end of life decision making. Differences relating to end of life decision making exist between regions and these differences can often be attributed to cultural factors. Such cultures not only affect patients and their families but also the health care workers who make and carry out such decisions. (shrink)

Henrich et al. propose that humans are genetically equipped with learning mechanisms that enable them to acquire the preferences and beliefs related to economic prosocial behaviors. In addition to their cross-cultural data, they cite developmental evidence in support of this theory. We challenge Henrich et al.'s interpretation of the developmental data in a discussion of recent work which suggests that preferences for altruism and fairness may have an innate basis.

Background:Nurse managers are placed in a unique position within the healthcare system where they greatly impact upon the nursing work environment. Ethical dilemmas and moral distress have been reported for staff nurses but not for nurse middle managers.Objective:To describe ethical dilemmas and moral distress among nurse middle managers arising from situations of ethical conflict.Methods:The Ethical Dilemmas in Nursing–Middle Manager Questionnaire and a personal characteristics questionnaire were administered to a convenience sample of middle managers from four hospitals in Israel.Results:Middle managers report (...) low to moderate levels of frequency and intensity of ethical dilemmas and moral distress. Highest scores were for administrative dilemmas.Conclusion:Middle managers experience lower levels of ethical dilemmas and moral distress than staff nurses, which are irrespective of their personal characteristics. Interventions should be developed, studied, and then incorporated into institutional frameworks in order to improve this situation. (shrink)

Background:Cardio-pulmonary resuscitation is the default procedure during cardio-pulmonary arrest. If a patient does not want cardio-pulmonary resuscitation, then a do not attempt resuscitation order must be documented. Often, this order is not given; even if thought to be appropriate. This situation can lead to a slow code, defined as an ineffective resuscitation, where all resuscitation procedures are not performed or done slowly.Research objectives:To describe the perceptions of nurses working on internal medicine wards of slow codes, including the factors associated with (...) its implementation.Research design:This was a cross-sectional, descriptive study. Participants completed a personal characteristics questionnaire and the Perceptions and Factors of Slow Codes questionnaire designed for this study.Participants and research context:The sample was a convenience sample of nurses working on internal medicine wards in two Israeli hospitals.Ethical considerations:The study received ethical approval from both institutions, where data were collected and stored according to institutional policy.Findings:Most reported that resuscitations were conducted according to protocol. Some took their time calling the code, or waited by the bedside and did not perform cardio-pulmonary resuscitation. Factors most associated with slow codes were poor patient prognosis and a low chance of patient survival. Two-thirds reported that slow codes were done on their unit and the majority perceived slow codes as ethical.Discussion:This study confirms that slow codes are part of medical care on internal medicine wards, where most nurses perceive them as an ethical alternative. These perceptions are in contrast to most legal and ethical opinions expressed in the literature.Conclusion:Nurses should be educated about the legal and ethical implications of slow codes, and qualitative and quantitative studies should be conducted that further investigate its implementation. In recent years, there has been a growing public discourse regarding the influence AI will have on the future of work. Simultaneously, considerable critical attention has been given to the implications of AI on gender equality. Far from making precise predictions about the future, this discourse demonstrates that new technologies are instances for renegotiating the relation of gender and work. This paper examines how gender is addressed in news media discourse on AI and the future of work, focusing on Germany. (...) We approach this question from a perspective of feminist technology studies and discourse analysis, exploring a corpus of 178 articles from 2015 to 2021 from German newspapers and newsmagazines. The findings indicate that critical AI and gender knowledge circulates in public discourse in the form of specific discursive frames, thematizing algorithmic bias, automatization and enhancement, and gender stereotypes. As a result, we show that, first, the discourse takes up feminist and scholarly discourse on gender and discusses AI in a way that is informed by social constructivism and standpoint theories. Second, gender appears as a—to some extent intersectional—diversity category which is critical to AI, while at the same time omitting important perspectives. Third, it can be shown that there is a renegotiating of the ideal worker norm taking place, and finally, we argue that the gendered frame of the powerful men developer responsible for AI’s risk is a concept to be challenged. In writing about `whiteness' we are trying to enact a `way of talking' that draws in part on Aboriginal ideas about how to conduct a conversation or tell a story. We also use Homi Bhabha's ideas of `third space' (an `interruptive, interrogative, and enunciative' space) and hybridity as a related way to think through the problems of essentializing binaries and rigid identities. In Aboriginal cultures in Australia and Canada, rather than adopting the `neutral' or `objective' stance common in the academy, (...) it is customary to introduce oneself to one's audience, providing a context to assist in interpretation and exchange. Without such an introduction, real stories cannot be told and productive conversations cannot happen. We thus begin our conversation with each other and with you by examining our personal relationship to the idea of whiteness in order to reveal some of its complexity in Canada and Australia. `Whiteness' as an abstraction has proved useful in moving the invisible norm to visibility, but we show how an awareness of `whiteness' in the two locations can be recuperated to re-privilege the already privileged. Aboriginal speakers and writers have theorized `whiteness', in many cases from outside the academy, in the process `hybridizing' traditional genres. For many of them, Aboriginality, like whiteness, is a construct that often stands in the way of thinking clearly about where to go next in the fight against racism. (shrink)

Patients with advanced dementia are less likely than those with other terminal illnesses to receive palliative care. Due to the nature and course of dementia, there may be a failure to recognize the terminal stage of the disease. A possible and under-investigated explanation for this healthcare disparity is the healthcare practitioner who plays a primary role in end-of-life decision-making. Two potential areas that might impact provider decision-making are cognitive biases and moral considerations. In this analysis, we demonstrate how the cognitive (...) biases and moral considerations of practitioners related to clinical decision-making are inherent in clinical practice and may impact on providers’ accuracy related to diagnostic and treatment related decision-making associated with patients with advanced dementia. Anchoring, default, availability, representativeness and framing biases are cognitive biases based on the "Two System Model" that relate to decision-making in end-of-life care. In patients with advanced dementia, those biases may result in a tendency to adhere to traditional mandatory care, involving an aggressive approach to care, which values saving lives at all costs, without taking into account the possible suffering and long-term consequences. Aspects such as moral sensitivity and moral courage play an important role in ethical decision-making related to advanced dementia. Investigations of clinical decision-making that include the cognitive biases and ethical considerations of practitioners might advance the comprehensive understanding of the clinical decision-making process related to care of patients with advanced dementia and promote the quality of care given to this population. Nach einer Reihe alleinstehender Personen wie Derbolav, Bühler, Jost, Glinz behandeln wir die Strömungen der symbolischen Logik und der linguistic analysis. Die symbolische Logik will das Denken in einer logischen Modellsprache analysieren. Die linguistic Analysis betrachtet die gewöhnliche Umgangssprache als Trägerin des Denkens und will, daß alle Philosophen und Wissenschaftler sich in dieser gewöhnlichen Umgangssprache ausdrücken, die bei guter Analyse für jeden intelligenten (...) Menschen verständlich ist. Die Strömungen der Phänomenologie und des Existentialismus äußern die Erfahrung, daß der Mensch seine Existenz ganz in einer sprechenden Welt erfährt und daß sich seine Existenz in der Sprache vollzieht : die Sprache ist das Haus seines Daseins. Keine Sache kann in unserem Bewußtsein zur Existenz gelangen, wenn wir sie nicht zu Wort bringen können. Die Ethnolinguisten und Psycholinguisten sind zu dem Ergebnis gekommen, daß die Formen, in denen ein Mensch jeweils denkt, von den unerbittlichen und unentrinnbaren Strukturen der Wörter mit ihren Bedeutungen und den Strukturen der Wortkombinationen kontrolliert werden. Diese Strukturen sind die Strukturen der jeweiligen Muttersprache. Das Denken eines Menschen vollzieht sich in seiner Sprache. Die Strukturen der Sprache liefern die Modelle für das Denken, für die Analyse der Eindrücke. Unser ganzes Denken bewegt sich in einer denkerisch-sprachigen Welt. Jedes Denken muß sich in Sprachstrukturen kanalisieren lassen. Die Sprache ist das formgebende Organ des Denkens. Die Denkanalyse vollzieht sich in Sprache. In einem bestimmten Augenblick der Entwicklung des Kindes begegnen sich Sprache und Denken ; dann wird das Denken „sprachig” und das Sprechen „denkerisch”. Von nun an werden die meisten Reize der physischen Welt durch ein sprachiges zweites Reizsystem gefiltert ; die Reize werden „in Sprache übersetzt”. Dieser Sprachfilter ist auch in der Analyse nach der dynamischen Erfassung der Reize vorhanden. Diese Daten aus den Theorien der Linguistic Analysis, der Phänomenologie, des Existentialismus, der Ethnolinguistik und Psycholinguistik kann man in direkte Verbindung bringen mit der neoklassischien Theorie, die über ein Erfassen von Erkenntnis-Daten in einer Intuition spricht, die implizit die Möglichkeit zu einem Explizitieren und Analysieren enthält. Scientific description of Nature is here based on geometry, number and energy. Geometry and number are the two only forms of our mind by which we describe Nature. Energy is here considered as the ultimate entity, which in physics is defined by the help of six propreties. The author holds that for an adequate description of physical Nature seven propreties of energy are required and eight are necessary in biology adding the holistic tendencies. On this basis an attempt is made (...) to show that holism is a scientific theory. As we can only perceive geometry and number, holistic propreties as such cannot be dealt with, we can only study the underlying physico-chemical manifestation. There are three holistic principles: 1) Interaction of parts leads to neocreation of propreties; this interaction can only be energetic. 2) The propreties of the whole are not potentially contained in the propreties of the parts. 3) The whole is more than the sum of its parts. Close interaction means fields of force and organisation, which implies that holism is organismic. The second principle is a safeguard against all unwarranted extrapolation. The whole and its parts only have additive propreties in common. Thus it is impossible to derive a macroscopic deterministic world from a microscopic undeterministic world because determinism is additive. The third principle is opposed to all attempts to describe holisation on a logical or mathematical basis. There is holisation in time and holisation in space and the latter can be subdivided into three groups according to their type of wholeness. Energy acts on energy and not on wholeness. All energetic interference with the basis has a holistic repercussion. Es gibt drei holistische Prinzipien: 1) Gegenseitige Beeinflussung führt zu Ganzheitseigenschaften. Beeinflussung kann nur energetisch sein. 2) Die Eigenschaften der Ganzheit sind nicht potential in den Eigenschaften der Teile enthalten. 3) Das Ganze ist mehr als die Summe seiner Teile. Gegenseitige Beeinflussung bedingt Kraftfelder und diese wieder Organisation. und so ist denn der Holismus seinem Wesen nach organismisch. Das zweite Prinzip legt Verwahrung gegen unstatthafte Extrapolation ein. Das Ganze und seine Teile haben nur additive Eigenschaften im gemein. Es ist also unmöglich eine deterministische makroskopische Welt von einer undeterministischen mikrophysischen abzuleiten weil Determinismus additiv ist. Das dritte Prinzip verwehrt sich gegen alle Anwendung von Logik und Mathematik zur Erklärung oder Beschreibung von Holisation . Es gibt Holisation 1) in der Zeit, 2) im Raum. Im Raum unterscheiden sich die Holisationen nach dem Wirkungskreis der Kraftfelder oder durch Abwesenheit solcher. Dies ist eine neue Aufteilung der Welt nach dem Wesen der Ganzheit. Die Einwirkung der Energie auf die Ganzheit ist mikrophysisch. Energie wirkt auf Energie und nicht auf die Ganzheit als solche. Jede energetische Einwirkung besitzt jedoch eine holistische Auswirkung. Das Problem der Eizelle und der epigenetischer Entwicklung wird auch besprochen. (shrink)

Dessen Freude an der Mathematik und Fähigkeit, «Figuren und Axiome der euklidischen Geometrie auf theologisch-philosophische Begriffe als verdeutlichende Gleichnisse anzuwenden, scheinen der Mentalität des Spieles sehr nahe zu stehen». The editors have attempted to invite contributions that would be interesting to both philosophers and social theorists. The conjunction of philosophic and sociological perspectives might help to overcome some of the mutual misunderstandings that have been fostered by a lack of dialogue between the philosophic and sociological action theory. Identitat ist unter den Bedingungen der Postmoderne kein festes Ganzes mehr. Die unuberschaubare Anzahl an Moglichkeiten in Ausbildung, Studium, Beruf, aber auch Partnerschaft und Lebensformen fuhren zu der Einsicht, dass Identitat sich immer neu konstituieren muss. Tatsachlich ist Identitat das, was mich als unverwechselbares Ich korperlich und geistig - und das heisst: leiblich - konstituiert. Zugleich entzieht sie sich mir und muss immer wieder neu errungen werden. Dies gilt gerade dann, wenn fruhere Ideale zusammenbrechen oder das zuvor Selbstverstandliche nicht mehr (...) tragt. Identitat und Identitatsbildung betreffen den Einzelnen ebenso wie politische oder religiose Gemeinschaften. In Zeiten, in denen vieles in Bewegung ist, ist Identitat eng verbunden mit der Frage nach den gemeinsamen Werten - das, wofur es sich zu leben lohnt und was wir an die nachste Generation weitergeben mochten. Besondere Beachtung verdienen in diesem Kontext phanomenologische Ansatze, die den leiblichen, sinnlichen und affektiven Dimensionen des Lebens Rechnung tragen. Auf diese Weise lassen sich Erfahrungen, die unsere Identitat begrunden und ausmachen, als Momente einer breiter angelegten Spiritualitat der Wahrnehmung verstehen. Cet article est divisé en quatre parties. Dans la première je cherche à prouver que la distinction cartésienne entre l'âme et le corps repose sur une base strictement objective, à savoir sur la transparence du doute et la possibilité de représentation du corps. Dans la seconde partie, je tente de montrer comment le doute qui suffit à prouver mon existence selon Descartes, doit se transformer, en raison meme de sa soi-disant transparence, en un etat nécessaire de cette meme existence. Nicht durch theologische Dogmen, nicht durch juristische Funktionalität, sondern allein im politischen Kommunikationsprozess wird Handlungsfähigkeit für die Gestaltung der Lebensumstände gewonnen und können Normen in einer kontingenten Welt verwirklicht werden. Althusius proklamiert am Vorabend des 30-jährigen Krieges die ars politica als Orientierungswissenschaft im Machtvakuum, das das konfessionelle Zeitalter in ganz Europa hinterlässt. In der Dichotomie von Nutzen und Notwendigkeit gilt die communicatio mutua als zentrale Bedingung menschlicher Vergemeinschaftung und wird zum komplexen Ordnungsgefüge im Staat ausgebaut. In Abgrenzung zur Reichspublizistik (...) handelt es sich bei der Politica um eine originelle Gesamtdoktrin, die sich überdies einer Einordnung in den schematisierenden Kontraktualismus der Neuzeit verschließt. Die Verankerung dieser Kommunikation in Strukturen, Organisation und Verfahren wird soweit betrieben, dass sich bereits am Beginn der Neuzeit eine institutionalisierte Penetration des Politischen in alle Lebensbereiche abzuzeichnen beginnt. Gegenüber den Aufsatzsammlungen zu einzelnen Aspekten der althusischen Lehre wird mit der vorliegenden Arbeit eine kohärente und konsequente Kommentierung der gesamten Politik versucht. Die Untersuchung reflektiert vor diesem Hintergrund sämtliche politische Symbiosen und Konsoziationen und setzt sich umfassend mit den althusischen Grundannahmen politischer Ordnung auseinander. Nietzsche hat keine politische Theorie vorgelegt und dennoch ganz grundlegende, auch heute noch herausfordernde Fragen zu Wesen und Wert des Politischen gestellt. Unter den Gesichtspunkten von Psychologie und Moral, Kultursteigerung und Zukunftsgestaltung hat er das Politische umgedacht und umgewertet. Ziel des Bandes ist, diese Umwertungen neu zu durchdenken. Dabei steht zunächst das Verhältnis von Nietzsches Philosophie zu den wesentlichen Fragen der Politik – Herrschaft, Gewalt, Freiheit, Selbstbestimmung und Gerechtigkeit – im Vordergrund. Die Rede von dem Konstruktivismus legt auf den ersten Blick die Vermutung nahe, es handele sich dabei um ein konsistentes, klar umrissenes Theoriegebäude. Doch wird auf den zweiten Blick schnell deutlich, dass ein ganzes Bündel von Referenztheorien zum Verständnis des Konstruktivismus in Betracht gezogen werden muss: etwa Evolutionstheorie, Neurobiologie, Kognitionstheorie, Kybernetik und Systemtheorie, um nur einige zu nennen. Es liegt auf der Hand, dass es den Konstruktivismus so nicht gibt.

The author deals with the meaning of metamerism. Two contrary points of view may serve to clear the problem. From the morphological point of view probably outgrowth of the organism in longitudinal direction has led to the development of metameres. So the first segmentation is due to a functional differentiation. In Phylogeny this segmentation has been maintained and the originally developed metameres remain the primary morphotic units of the organisms. Further specialising of function and organ-concentration destroyed the original absolute metamerism (...) and only left a rest of the original.The functional point of view renounces an original absolute metamerism for the present moment. It only asks for the functional meaning of the segmentation in organisms, showing this segmentation. From the functional point of view it is possible that metamerism may be the morphological expression of quite different functional requirements and that in one organism the metamerism starts from another system of organs than in another.Es wird gefragt welche Bedeutung die Metamerie haben kann. Zur Erläuterung des Problems werden zwei extremen Betrachtungsweisen einander gegenübergestellt. Nach der morphologischen Betrachtungsweise hat Wachstum der Organismen in die Länge wahrscheinlich zur Ausbildung der Metamerie geführt. Diese erste Metamerie ist wahrscheinlich ein funktioneller Differenzierungsprozess gewesen.In der Phylogenese hat sich diese Metamerie behauptet und die ursprünglich entstandene Metamere sind die primären morphotischen Einheiten der Organismen geblieben. Weitergehende Funktionsspezialisierung und Organkonzentration hat die ursprüngliche absolute Metamerie vernichtet. Was davon übrig blieb sind die Reste der ursprünglichen.Die funktioneile Betrachtungsweise verzichtet vorläufig auf eine ursprüngliche absolute Metamerie. Sie fragt bloss nach der funktionellen Bedeutung, welche die Metamerie besitzt bei denjenigen Organismen, wo diese gefunden wird. Sie achtet es möglich, dass Metamerie der Ausdruck sein kann von ganz verschiedenen funktionellen Anforderungen und dass somit die Metamerie bei der einen Tiergruppe von einem ganz anderen Organsystem ausgeht wei bei einer anderen. The essay attempts to point out the benefits of reading Peter Sloterdijk’s works from the perspective of German studies. In the course of the paper, Sloterdijk is introduced in his various individual roles. First, his early steps in German studies are described, followed by an assessment of the influence of his German-studies works on the formulation of philosophical problems and stances. After that, Sloterdijk is presented as an exponent of certain typical development, as a diagnostician of problems that post-war Germany (...) faced in its transformation into a civic society. The final part consists of reflections on his observational abilities in descriptions of interdisciplinary misunderstandings and in the creation of transdisciplinary analogies. (shrink)
